2025 Medicaid Changes & Cluster Care

With the September 2025 CDPAP overhaul requiring stricter ADL thresholds (3+ ADLs required), Cluster Care efficiently serves the new, smaller pool of clients with higher functional impairment—especially those with 5+ ADL/IADL limitations where cost savings are greatest.

This model aligns perfectly with New York State's efficiency mandates while ensuring quality care for those who need it most.
